Mycobacterium tuberculosis (TB) is an obligate aerobe that stains acid-fast positive. It lacks flagella but has pili, and while it possesses a microcapsule, it does not form spores. Its cell wall contains neither the teichoic acids of Gram-positive organisms nor the lipopolysaccharides typical of Gram-negative bacteria. Pathogenic strains are categorized into human, bovine, and African types. Disease results from inflammation triggered by bacterial multiplication within tissues, toxicity of bacterial products and metabolites, and immune-mediated damage to bacterial constituents. Key virulence factors include the capsule, lipids, and proteins. The bacillus typically enters a susceptible host via the respiratory tract, though digestive and cutaneous routes are possible, causing tuberculosis in various organs, with pulmonary infection being most common. Primary infection is seen mainly in children and is marked by low-grade fever, night sweats, and scant hemoptysis. Secondary disease, often in adults, presents with similar symptoms but can have an acute onset. Tuberculosis ranks among the top ten global causes of death. In 2018, about 10 million people worldwide became infected, and approximately 1.6 million died. China is a high-burden country, with the second highest incidence rate globally.
