Epidemiology

Chronic myelogenous leukemia (CML) arises from a malignant clonal expansion of hematopoietic stem cells. Over 95% of individuals with CML harbor the Philadelphia chromosome (Ph) in their blood cells. The primary pathogenic mechanism involves formation of the BCR-ABL fusion gene through a reciprocal translocation between the abl proto-oncogene (Abelson murine leukemia viral oncogene homolog 1) at chromosome 9q34 and the breakpoint cluster region (BCR) gene at 22q11. The resulting fusion protein possesses tyrosine kinase (TK) activity and triggers downstream signaling cascades—including RAS, PI3K, and JAK/STAT pathways—that drive cell proliferation and suppress apoptosis, leading to malignant cell growth and CML development. The BCR-ABL fusion is a key diagnostic marker for CML. Moreover, monitoring changes in its transcript levels serves as a dependable indicator for assessing prognosis and predicting leukemia relapse following therapy.
