CYP2C19 belongs to the cytochrome P450 family and plays a key role in drug metabolism. It processes numerous endogenous compounds and approximately 2% of clinically used drugs, including antiplatelet agents (e.g., clopidogrel), proton pump inhibitors (omeprazole), and anticonvulsants. Genetic variation in CYP2C19 leads to altered drug-metabolizing capacity. The *2 (rs4244285) and *3 (rs4986893) loss-of-function alleles reduce enzyme activity and substrate metabolism, raising plasma drug levels and increasing the risk of concentration-dependent adverse reactions. In contrast, the *17 (rs12248560) allele enhances CYP2C19 activity, boosting active metabolite formation, which can intensify platelet inhibition and elevate bleeding risk. Individuals with poor metabolizer status who receive standard doses over prolonged periods may experience severe toxicities, including hepatotoxicity, hematopoietic damage, and central nervous system injury, potentially leading to fatal outcomes. Based on metabolic phenotype, patients are classified as: ultrarapid metabolizers (UM: *17/*17, *1/*17), rapid metabolizers (RM: *1/*1), intermediate metabolizers (IM: *1/*2, *1/*3), and poor metabolizers (PM: *2/*2, *2/*3, *3/*3).

Work Flow

For sample preparation, a universal DNA/RNA kit may be employed (compatible with an automatic nucleic acid extractor). Follow the manufacturer's protocol, using 200 µL of sample and an elution volume of 100 µL.

Alternatively, a genomic DNA purification kit (catalog number A1120) or a nucleic acid extraction/purification reagent (YDP348) can be used. Adhere to the respective extraction instructions, with a recommended input volume of 200 µL and an elution volume of 160 µL.

SpecificityThere is no cross-reactivity with other highly consistent sequences (CYP2C9 gene) in the human genome. The mutations of CYP2C19*23, CYP2C19*24 and CYP2C19*25 sites outside the detection range of this kit have no effect on the detection effect of this kit.
