Warfarin is an oral anticoagulant widely employed in clinical settings for the prophylaxis and management of thromboembolic conditions. It possesses a narrow therapeutic index and displays substantial inter-ethnic and inter-individual variability. Data reveal that the stable dose requirement can differ by more than 20-fold between patients. Annually, 15.2% of patients on warfarin experience bleeding adverse events, with 3.5% developing fatal hemorrhages. Pharmacogenetic research indicates that polymorphisms in the warfarin target enzyme VKORC1 and the metabolizing enzyme CYP2C9 significantly contribute to dose variation. Warfarin acts by specifically inhibiting vitamin K epoxide reductase (VKORC1), thereby blocking vitamin K-dependent clotting factor synthesis and exerting its anticoagulant effect. Numerous studies highlight that VKORC1 promoter polymorphisms are the primary determinants of racial and individual differences in warfarin dosage requirements. Warfarin is metabolized by CYP2C9, and mutations in this gene markedly slow drug metabolism. Consequently, patients on warfarin face a heightened risk of bleeding during the initial phase of therapy, estimated to be two to three times greater.
